At River Tyne Academy Gateshead, we place the individual needs of each student at the centre of our reintegration and transition planning. Whether a young person is returning to mainstream education, moving into specialist provision, or progressing to a post-16 destination, we are committed to ensuring that every transition is carefully planned, well-supported, and successful. 

We recognise that transitions can be a significant step for students and families. For this reason, we prioritise clear communication and close collaboration with all involved, including the student, parents and carers, the receiving school or setting, and any relevant professionals. 

Bespoke Transition Planning

Each student benefits from a personalised reintegration or transition plan that is co-produced with key stakeholders. This collaborative approach ensures that individual strengths, needs, and aspirations are fully considered and that support is tailored appropriately. 

Plans are regularly reviewed to monitor progress, celebrate successes, and identify any emerging barriers. This ensures that support remains responsive and effective throughout the transition process. 

Our Approach to Successful Reintegration

Our aim is to ensure that every student feels confident, supported, and prepared for their next step. Working in partnership with receiving schools and settings, we provide a structured and consistent approach that may include: 

  • Initial Support and Engagement
    Early collaboration with the receiving school to ensure a shared understanding of the student’s needs and strengths from the outset. 
  • Clear Goals and Expectations
    Jointly agreed targets, timescales, and success criteria that are clearly communicated to all stakeholders, including the student and their family. 
  • Reasonable Adjustments
    Identification of appropriate adjustments and strategies to support a successful and sustainable transition. 
  • Specialist and Targeted Support
    Access to tailored support from River Tyne Academy Gateshead staff, which may include pastoral input, personal development work, literacy support, or emotional wellbeing interventions. 
  • Key Worker Support
    Each student is supported by a designated key worker who acts as a consistent point of contact, providing guidance and coordination for the student, family, and receiving setting. 
  • Ongoing Transition Support
    Continued support is available during the early stages of reintegration to help ensure stability, confidence, and long-term success.
